Rapidly progressive post-transplant lymphoproliferative disease following withdrawal of sirolimus.

چکیده
Sirolimus, a potent inhibitor of B- and T-cell activation. is a commonly used immunosuppressant after renal transplantation. Withdrawal of sirolimus from the immunosuppression regimen may reduce B-cell surveillance. We present a case of rapidly progressive central nervous system (CNS) polymorphic Epstein-Barr virus (EBV)-related post-transplant lymphoproliferative disorder following the withdrawal of sirolimus.

Resolution of renal allograft-associated post-transplant lymphoproliferative disorder with the introduction of sirolimus.
Lymphoid neoplasia has remained a serious complication following solid organ transplantation. Recent data suggest an 11.8-fold higher risk of lymphoma compared with the general population over a 10 year period [1]. Although the risk is greatest within the first year posttransplant, the incidence remains high throughout the entire post-transplant period. Risk factors include unrestrained prolife...
متن کاملRituximab Is Indispensable for Pediatric Heart Transplant Recipients Developing Post transplant Lymphoproliferative Disorders
Abstract Rituximab, an anti-CD20 agent, has been suggested as an effective strategy to deal with post transplant lymphoproliferative disorders (PTLD). In the current study, we aim to evaluate the efficacy of rituximab therapy in heart transplant population developing PTLD. A comprehensive search of the literature was performed to gather the available data on lymphoproliferative disorders oc...

In orthotopic liver transplantation (OLT), tolerance is induced in a certain combination of donors and a recipient in rats and, in some clinical cases, rejection has not occurred in OLT patients after weaning off immunosuppression. However, this mechanism has not yet been elucidated. Among our cases of liver transplantation (LTx), one OLT patient (Patient A) has not required immunosuppressive d...
متن کاملPosttransplantation lymphoproliferative disorder of the paranasal sinuses mimicking invasive fungal sinusitis: case report.
Posttransplantation lymphoproliferative disorder (PTLD) is a challenging complication associated with organ transplantation and is usually fatal if untreated. We describe the case of a transplant recipient who presented with rapidly progressive cranial nerve palsies due to PTLD that originated in the sphenoid sinus. In this case, the clinical and radiologic presentation of PTLD mimicked invasiv...
متن کاملEBV-associated recurrent Hodgkin's disease after renal transplantation
Hodgkin's disease is recognized as part of the spectrum of post-transplantation lymphoproliferative disorders (PTLD), although it is still an uncommon de novo malignancy in this population. Epstein-Barr virus (EBV) has been linked to both post-transplant non-Hodgkin's lymphomas and Hodgkin's disease. We report a case of recurrent Hodgkin's disease in a patient who received a renal transplant in...
